Session Description: This session explores a transformative value-based care initiative: the integration of an Orthopedic-Like Same Day Clinic within primary care settings, leading to a significant reduction in emergency department (ED) visits per month across a regional health system. Designed to address musculoskeletal complaints efficiently and cost-effectively, this model enhances access, improves patient outcomes, and supports care continuity—all while reducing unnecessary ED utilization and addressing overall cost of care concerns. The session will be co-led by representatives from Yakima Valley Farm Workers Clinic (YVFWC) , demonstrating the power of collaborative innovation between frontline staff and leadership. Attendees will gain actionable insights into how this model was implemented, the operational and clinical workflows involved, and the measurable impact on patient care and system costs.
