Session Description: With 11% of health center patients having Medicare coverage, health centers need to understand how to take full advantage of FQHC provisions in the Medicare Physician Fee Schedule and specifically, the advanced primary care management services to prepare for the transition towards value-based care. This session will share best practices and the latest updates for health centers preparing to incorporate Medicare beneficiaries into value-based care arrangements. Participants will be able to learn what’s on the horizon in Medicare policy and how to best provide and document all services they’re providing for their Medicare patients to better position themselves in the move towards value-based care arrangements

  • Understand the newly finalized care coordination codes and updates to advanced primary care management codes, among other regulatory changes.
  • Learn best practices on how to maximize documentation and billing for these Medicare services.
