Session Description: Community health centers are under mounting pressure from workforce shortages and constrained funding. AI-powered solutions offer a new way forward – automating time-intensive tasks so care teams can practice at the top of their license, improve care coordination and care management, and strengthen patient engagement in value-based care.

This session will highlight early lessons from a Medical Home Network pilot with Oklahoma PCA/CIN health centers (starting with Stigler Health and Wellness, Variety Care, and NEO Health), all participants in the ACO REACH model through a partnership with MHN. Panelists will share a practical roadmap for deploying AI solutions, including initial use cases and data, and strategies to enhance workforce efficiency and patient engagement and support transitions of care in value-based care.

An initial pilot use case focuses on Medicare Health Risk Assessments (HRAs), leveraging AI chatbots and telephonic agents to:
- Engage attributed patients via text or phone
- Simplify HRA completion through conversational AI
- Streamline workflows and close care gaps
